The Care Card is a simple and effective way for Heritage Valley Health System to uniquely identify you quickly and accurately.  We will provide you with two cards, a wallet size and a keychain size card. Either card will work at all our facilities supporting this service.  Carry the card that works best for you!

The Care Card will allow us to improve the speed and accuracy of your appointment, scheduling, and registration processes.  By presenting the Care Card to our registration personnel or at the kiosks, we will be able to quickly verify your information and register you and your family members more accurately.
