The Care Card is a simple and effective way for Heritage Valley Health System to identify you quickly and accurately. The Care Card and your Care Card number will uniquely identify you. We will provide you with two Care Cards, a wallet size card and a keychain card. Either card will work at all our facilities supporting the Care Card. Carry the card that works best for you!

The Care Card will allow us to improve the speed and accuracy of your appointment, scheduling, and registration processes.

By presenting the Care Card at the Kiosks in the following places, we will be able to more accurately register you and your loved ones:

Scanning your care card is as simple as scanning your shoppers cards at your local store.

Hold your Care Card from the edge.

The back of your Care Card (bar coded side) will look similar to this.

You will hear a tone when your card has been read.

The day of your exam please bring your Care Card and present it to at the registration coordinators desk. Using your card helps our registration personal quickly verify your information and get you to your destination in a timely manner.
