Working closely with the physiatrist is a highly trained group of rehabilitation specialists that include rehabilitation nurses, physical, occupational and speech therapists, psychologists, case managers and social workers.
The rehabilitation team recognizes that the primary care physician is important for the overall medical care of the patient. While the team is responsible for the patient's therapy during the program, the primary care physician follows the patient's medical care as often as the complexity of the patient's medical problems necessitate. The rehabilitation team provides reports and updates to enable the physician to follow up on the patient's progress.
Patients meet frequently with a social worker or nurse case manager who is the liaison between the patient, the rehabilitation team and the patient's family. The case manager provides guidance in obtaining information on a variety of services and resources including insurance coverage requirements, equipment needs in preparation for discharge and follow-up home care or outpatient therapy.
