According to the Centers for Disease Control and Prevention, there are an estimated 79 million adults aged 20 and older that have pre-diabetes. Others have been diagnosed with metabolic syndrome, a group of risk factors that include a large waist, unhealthy cholesterol, high blood pressure, and high fasting blood sugar. However, persons diagnosed with pre-diabetes or metabolic syndrome can take control of their condition and take steps to avoid developing diabetes.

They can learn to live smart with LifeSmart!

LifeSmart is a diabetes prevention program that is offered through a partnership between Heritage Valley Health System and UPMC Health Plan. It provides healthy lifestyle programs for adults within the Heritage Valley Health System community who have been identified as having risk factors for pre-diabetes or metabolic syndrome.

LifeSmart nurse educators and dietitians work with your personal physician to help you lead a healthier lifestyle that can possibly prevent diabetes. The four components of LifeSmart include the following:

• Group Lifestyle Balance (GLB) Program
• Individual Nutrition Education
• Smoking Cessation Classes
• Exercise and Fitness Opportunities

LifeSmart is provided at no cost. Participants must have a Heritage Valley Health System Physician, but do not need to have UPMC Health Plan benefits to participate.
