Patients leaving the hospital may need nursing care at home as they recuperate. Some may need to complete their recovery at a long-term care facility. Hospital stays are short, so, if possible, plan for these concerns before you are admitted.
One or more members of case management services (nurses and social workers) will help plan for care after discharge. They can link you with:

- Spiritual counseling
- Community education programs
- Home-delivered meal service
- Lifeline emergency response services
