All specifically identifiable clinical or social information collected from or about the patient in any form (written, spoken, recorded electronically or printed) is protected from accidental or intentional modification, destruction or disclosure.
It is the duty and responsibility of any individuals associated with HVHS who come in contact with such information to ensure that all patient information is used only for the purposes for which it is obtained, and will not be divulged except to the extent that disclosure is otherwise authorized or mandated by law or by third party contractual agreements.
