In partnership with Lutheran SeniorLife, Heritage Valley Health System is proud to sponsor LIFE Beaver County as an extension of its mission to address both the prevention and treatment of disease throughout the continuum of life. The program, which also serves as the participants’ health plan, is part of a national program called PACE (Program for the All-Inclusive Care of the Elderly), and is one of several LIFE programs operating in western Pennsylvania.
LIFE (Living Independence for the Elderly) Beaver County helps older adults continue living in their homes with comprehensive and coordinated services to address a range of needs that often accompany the aging process. LIFE also makes life easier for families and friends currently providing caregiving. The program lifts the burden of worry often felt by families by partnering with them to manage all of the complex medical, functional and social needs for their loved one. LIFE coordinates doctors’ appointments and nursing care. If family members and friends are sharing the responsibility for carrying out the over all program of care, they gain peace of mind knowing that their loved one has a better quality of life while living at home.
LIFE medical management services include the following:

An interdisciplinary team including a physician, nurse practitioner, nurses, social workers, dietician, recreational coordinator and rehabilitation therapists
- An oncall nurse and physician available 24 hours a day, seven days a week
- Individualized, personal care
- Support and assistance to caregivers and to family members involved in providing care
Based on an individual’s need, LIFE can provide the following services:
- Adult day services
- Personal care
- Homecare, housekeeping, and laundry
- Recreational programs
- Physical, Occupational and Speech Therapies
- Transportation to the LIFE center and to medical appointments
- Medication management
- Meals/Assistance with meal preparation
- Medical equipment
- Equipment for safety in the home
- Dentistry, Optometry, and Audiology
A day at the LIFE Beaver County center may include the following:
- Seeing the doctor
- Receiving medications
- Exercising
- Enjoying activity programs
- Having lunch with friends
- Participating in physical therapy
- Receiving assistance with personal care
LIFE also manages the participant’s medical care in a hospital, nursing home or other inpatient or outpatient setting by coordinating appointments and the services received.
A person is eligible for LIFE Beaver County if he or she is...
- 60 years of age or older
- having difficulty managing his or her own healthrelated needs independently at home and meets a level of care determined by the Beaver County Office on Aging.
- eligible for Medicaid or able to pay privately
- living in Beaver County
- able to live safely at home with our services
*LIFE enrollees must agree to receive all needed healthcare, including primary care and specialty physician services (other than emergency services), from LIFE Beaver County and its providers. A co-payment may be required for medications or nursing home care. LIFE Beaver County is affiliated with a nationally recognized program called PACE, the Program of All-Inclusive Care for the Elderly. PACE provides quality care and services for frail older adults who need support to stay independent in their own home. LIFE Beaver County is administered through the Pennsylvania Department of Public Welfare by VieCare, Inc; a subsidiary of Lutheran Senior Life and Heritage Valley Health System.
