Trinity Family Practice is a division of Heritage Valley Medical Group, part of Heritage Valley Health System.  Caregivers include four physicians, one nurse practicioner and one physician assistant.  We are committed to providing our community with the highest quality of health care by treating the whole patient, body, mind and spirit.

If at any time one of our patients needs to be admitted to the hospital, our physicians are on staff at Heritage Valley Beaver. 

Our office staff is available to answer the telephones from 8:30 a.m. to 4:30 p.m., Monday through Friday.  When you call, the phone will be answered by an automated attendant.  Please listen carefully to the options and direct your call accordingly.
