We provide internal medicine services to adult patients. Some of these services include:  Diabetes prevention and management, Heart disease prevention and management, High cholesterol prevention and management, Physical examinations, Preventative health care physicals, School and sports physicals, Treatment of high blood pressure, Treatment of lung disease and Treatment of arthritis.

We are able to perform many services in the office, such as: Age appropriate immunizations, Electrocardiogram, Ear cleaning, Laboratory services / blood draws and Vision and hearing screening. 

We like the personal touch. When you call our office, you will speak with a real person, not a phone tree. Because of this, we may have to ask you to hold the line briefly as we handle our incoming calls. The staff members who are taking your calls are medically trained personnel who will gather the necessary information to pass along to our physicians. 

Office hours are by appointment, Monday through Friday. We strive to see patients as close to their appointment times as possible. If we are delayed in seeing you, please understand that unforeseen situations can arise that require additional time.

If you are unable to keep an appointment, we ask that you notify us at least 24 hours in advance. Your appointment time can then be offered to another patient. There is a no show charge if you do not keep your appointment and have not canceled the appointment.

Plan to arrive at least 10 - 15minutes ahead of your appointment time, so we can register you into the computer and prepare you for your visit. When you arrive for your appointment, please sign in at the front window. Be prepared to show your insurance card each time you come in to ensure proper billing. To help protect your information against identity theft, we may also require your driver's license for positive identification.

Under normal circumstances, we will do our best to schedule your appointment with the physician you prefer to see. However, if you are ill or have a medical emergency and require immediate attention when your physician is unavailable, we will anticipate you to be willing to be seen by any one of our physicians. 

Fees and Insurance
We accept most insurance plans and will bill your insurance directly for plans we participate with. Payment for co-pays and non-insured services is expected when the service is rendered. We accept cash, personal checks and major credit cards. Payment arrangements can be made if necessary.

Managed Care Insurance Referrals
If our physicians have referred you to a specialist for medical care, your insurance company may require that a referral be processed by your primary care physician. We require a 5-7 business day notice to process all non-urgent referral requests. Without adequate notice to our referral department, non-urgent appointments may need to be rescheduled or may not be paid by your insurance company.

The length of your new patient appointment will be determined by your age as well as your medical history. Once we have scheduled your appointment, you will receive a new patient packet in the mail. In the envelope you will find a blue registration form and a 2 page confidential health history. These forms should be completed and brought with you to your first appointment.

Blue registration form: Please complete all appropriate sections on the front and the back. Please note your emergency contact should be someone who does NOT live with you that we may contact in the event of an emergency or if we are unable to reach you.

Confidential health history: Give yourself adequate time to complete these forms. They are quite detailed. The physician will review these forms with you during your visit to evaluate your medical history and your current concerns. 

For your safety and health, please inform the physician of any medication or change in medication that you take. To ensure accuracy, please either bring your medication bottles or a complete list of your medications to include the name, the dosage (mg) and how often you take the medication. Please also include any over the counter products you take.

Our physicians utilize electronic prescribing to send your prescriptions directly to your pharmacy. This technology will help ensure the safety and accuracy of your medications. You may request medication refills during your office visit or you may call the office if you need refills between your visits.

Routine medication refills will not be handled over the weekend by the doctor on call. 

One of our physicians is on call every evening and/or weekend for emergencies only. This would include life threatening circumstances or situations or sudden illnesses that might require immediate medical care. Therefore, calls regarding routine medication refills or questions that do not require immediate assistance should be made to the office during regular business hours. 

Physicals required for pre-employment, school (sports or enrollment), camp, adoption, etc.
Physicals that are administrative in nature, meaning they are required to satisfy the requirements of a company, school or organization, are not routinely covered by your health insurance. Because of this, we will not submit a claim to your health insurance company and you will be required to pay the associated charges at the time the service is rendered. In consideration of this fact, our fee for this service is reduced. Any immunizations or exams required in addition to the physical exam would incur additional expenses. Zvonar, M.D. are Board Certified by the American Board of Internal Medicine, certified by the NCQA for excellence in the management of diabetes and maintain ongoing continuing education to stay up to date with the most current treatment plans available.  

Office visits are by appointment only to patients 18 years of age and older. We participate with most major insurance plans.
