Community Advanced Illness Program (CAIP)
What is CAIP?
The CAIP helps patients and families cope with a chronic illness by addressing the needs and concerns that arise when one is chronically ill. The CAIP enhances quality of life by focusing on choice, quality care, satisfaction and peace of mind. CAIP services are free and open to individuals residing in Beaver and western Allegheny Counties.
A CAIP Care Coordinator will:
- Coordinate care with the individual’s healthcare provider.
- Assist individuals/caregivers in accessing supportive services needed to manage on a day-to-day basis, i.e. in home services, transportation, and medical equipment.
- Discuss choices regarding advanced care planning, including advance directives.
- Offer support and guidance to the individual and family members.
