At Family Practice Associates Sewickley, we are committed to caring for patients in all phases of life, from newborn to elderly. We provide routine health maintenance and preventive care, routine woman care including contraception and PAP smears, treatment for both acute and chronic illnesses, and certain surgical procedures such as cryotherapy for wart removal or incision and drainage of an abscess.

We consider immunizations to be an important part of health maintenance throughout a person’s life. We recommend immunizations based on the guidelines from the Centers for Disease Control.

We can perform immunizations or other injections (such as allergy shots) at the time of your doctor visit, or you may receive them during our injection hours.  Please see the “Location/Hours” tab.

If a specialist is needed for participation in your care, we will refer you to trusted colleagues. We endeavor to communicate openly with the specialist(s) in order to provide the best possible care for you. Most insurance plans do not require a formal referral. 

We will gladly fill out most forms. Most commonly, we are asked to fill out sports, work, and drivers license physical forms. Please bring these to your appointment if possible. In certain rare circumstances, for example a very lengthy form, a fee may be charged.

Leave all requested information, including:

  • Your name and date of birth
  • The spelling of your medication
  • Dosage and instruction for taking (example: 40mg tablet, one tablet twice a day)
  • Name, address and phone number of the pharmacy
  • For a 30-day supply, please call at least one week before you will run out of your medication.
  • For a 90-day supply, please call at least two weeks before you will run out of your medication.
  • For all mail-order pharmacies: If we are provided with a completed form, we can fax this form along with your prescription. We do not call or mail the prescription to the mail order pharmacy.
  • Please note that routine prescriptions and prescriptions for narcotic medications will not be ordered after business hours or on weekends. 
  • Call 412.749.6801 and leave all pertinent information.
  • A routine referral will be processed within seven business days. Emergent referrals will be processed the same day.
  • Please note that most insurance companies do not require a referral. Please check with your insurance company to find out whether one is needed. 

We routinely call patients with any abnormal test results.
If you want to hear about your test results, please follow these instructions:

  • Call our office 1-2 weeks after your tests were completed
  • Call 412.749.6801. Please leave all requested information, including:
    • Your name and date of birth
    • Type of test
    • Facility where test was performed
    • We will get back to you within the next 1-2 business days. 
  • Patients who are currently under our care should come in at least once a year for a routine visit, or more frequently if recommended by their physician.
  • When ill, patients must be seen by a physician. Please do not call requesting medications for an acute illness.
  •  Patients arriving past their scheduled appointment time may be asked to reschedule their appointment.
  • Please bring each of the following with you at each visit:
    • Insurance card
    • All prescription and over-the-counter medications, including vitamins and herbal supplements, in their original bottles
    • Co-payment
    • All fees are due at the time of the visit. We do not bill for co-payments. We accept cash, checks, debit and credit cards (including Visa, MasterCard, and Discover).
  • After normal business hours, one of our physicians is on call.
    • In the event of a life threatening emergency, call 911 or proceed directly to the nearest Emergency Department.
    • Please restrict your after-hours calls to emergency situations only.
