When calling for an appointment please dial 724.773.5833 and press “4” for an operator. So that we schedule the appropriate amount of time needed for your problem, please be prepared to provide us with the reason for your appointment.
Patients with emergent issues will be seen immediately or referred to the Emergency Department. A non-urgent appointment will be scheduled within 3 to 5 days.
We participate in most major insurances. Co-pays and non-covered services are due at the time of service (we accept checks, credit, and debit cards), with no exceptions. We ask that you bring your current insurance information, a picture ID, and your Heritage Valley Care Card to each appointment for HIPAA compliance and proper billing. Please provide a list of your current medications ( including over-the-counter medications, vitamins and supplements).
Always inform the operator of any address, email, and phone number changes.
If you are a new patient, we welcome you. Please print out our “New Patient Forms” as well as a HIPAA privacy form to review and complete in advance-- Please bring them with you to your appointment. Should you have any further questions or concerns please feel free to contact our office.
No Show Fee: Cancellation of appointments must be 24 hours in advance to avoid a no-show fee. The fee is $25 for established patients and $50 for new patients. After 3 cancellations we reserve the right to discharge you from our practice.
For refills during your office visit please bring a list of your current medications (including over the counter medications) with the dosing and directions. Medications will be reviewed at each visit. Please let the nurse know of any changes. Medications are sent to you pharmacy electronically in most cases. Please note that if you use a mail order pharmacy for any controlled medications it may take a little longer to receive that medication as electronic signatures are not permitted and the doctor's original signature needs to be sent over additionally.
For refill requests please note that we try to fill all prescriptions the same day, however, it can take up to 2 business days so please call 5 business days in advance to guarantee your medications are filled on time.
We do not refill prescriptions after hours or on the weekends.
When requesting a refill dial our main number (724.773.5833), choose option 2 and a recording will prompt you.

When requesting a referral authorization please allow 48 hours to process your request.
When calling please be prepared with the following information
- Your name
- Insurance name and ID number
- Date of appointment
- Reason for the appointment
- Doctor or facility name where the services are to be rendered
Test results are not immediately available. Turn around time varies depending on the type of testing, where the testing was done, and when the results are reviewed by the physician.
Abnormal results will be communicated by phone. Please allow at least 7 business days before contacting us. If you have an appointment scheduled within 4 weeks the physician will review your results at the time of your appointment. If it is felt that a sooner appointment is warranted you will be contacted.

Our office accepts most insurance programs. If you should have any questions or concerns as far as coveage, please contact your member services. They will be able to answer your questions.

Our Physicians are Board Certified and NCQA certified. We also have Elizabeth Salyards, CRNP on staff to serve your medical needs. She is a member of the American Academy of Nurse Practitioners and has been a part of our team since 1994.
