First and second doses of the Pfizer COVID-19 vaccine are available for children ages 5 – 11 by appointment at the locations below. Vaccine appointment days and times vary. Walk-in times are not available for vaccines and boosters. A consent form, to be signed by a parent or guardian, is required and will be provided at the time of the appointment. Please note that a separate appointment is needed for each child in a family.

COVID-19 Boosters
Heritage Valley is offering Pfizer and Moderna booster vaccines by appointment to all adults, ages 18 and older, who received the Pfizer BioNTech vaccines at least five months ago or Moderna vaccines at least six months ago. Adults who received the vaccine from Johnson & Johnson more than two months ago are also eligible for boosters.
Children 12 – 17 years old who received Pfizer vaccines can get a Pfizer BioNTech COVID-19 vaccine booster, by appointment, at least five months after their primary COVID-19 vaccination series.
People ages 12 years and older who are moderately or severely immunocompromised may choose to receive a second booster dose at least four months after the first booster dose. (Moderna boosters may only be given to individuals ages 18 and older.)
Adults ages 50 years and older who are not moderately or severely immunocompromised may choose to receive a second booster dose at least four months after the first booster dose. Pfizer and Moderna boosters are currently being provided at the locations below. Walk-in times are not available for vaccines and boosters.

If you received Pfizer 1st & 2nd vaccines, we will only administer a Pfizer booster.
If you received Moderna 1st & 2nd vaccines, we will only administer a Moderna booster.
If you received a Johnson & Johnson vaccine, you may schedule either a Pfizer or Moderna booster.
Your Vaccination Record Card will be updated at your booster appointment.
If you did not receive your first and subsequent vaccines from Heritage Valley, you must bring your Vaccination Record Card to your booster appointment, as proof of vaccines is required.
