Our History
The physicians of the Association of Specialty Physicians, Inc., now
HVMG Orthopedics, have serviced the Beaver County and surrounding areas for many years prior to their merger in July, 1996. They had operated as individual practices but decided to merge their practices so that they could share their mission and resources to better serve the community. They have continued to grow in their recognized leadership in providing orthopedic, general surgery (including vascular and thoracic), and urology services and continue to have additional physicians join the group since its inception.

Our Mission
HVMG Orthopedics is a Board Certified surgical specialty group dedicated to treating people who are seeking medical care in a timely, professional manner, upholding the highest standard of care. Patient satisfaction, restoration of health and efficient utilization of resources is our focus. We will promote dignity, recognition, and pride in service with our employees so they may participate in delivering quality service in a professional atmosphere.

Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.

Emergency
If you have a true medical emergency after office hours, please call 724.775.4242.

New Patients
If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:

  • Driver’s license or state ID
  • Referral (if required by an insurer)
  • Insurance information
  • A list of current medications and any known allergies
  • Copies of operation records, medical records, x-rays, MRIs, and CT scans from prior doctor visits

Insurance Information
We accept most major insurance plans. Please remember to bring your insurance card with you to your appointment. Co-payments are due at time of service.

Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, and Discover for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient’s responsibility. We will help in any way we can to assist you in handling claims.

Workers Compensation
In order to bill your workers compensation claim, we need the full name and address of the workers compensation carrier, date of injury, “Employer’s report of occupational injury” and address and phone number of your employer and contact person.

Disability and Insurance Forms
There is a $20.00 charge for most disability and insurance forms to be completed.

Billing Office
Should you have questions concerning your bill from the practice, please call 724-888-5688 or 800-900-1377.

Please request all prescriptions and authorizations for renewals between 8:00 a.m. – 3:00 p.m., Monday to Thursday and 8:00 a.m. – 12:00 p.m. Friday.

You may need a referral from your Primary Care Physician (PCP) to see one of our physicians. It is the patient’s responsibility to get this referral. Please refer to your insurance booklet you received from your carrier or employer. Also, there may be a phone number listed on your insurance card for contact information about your insurance plan. If you do not have a required referral, we will need to reschedule your appointment.
