New Patient Packet

We accept most insurance plans. If you are unsure if your plan will cover you in our office you may call the member service number on the back of your card to see if we are a participating provider.  Please be sure to notify us of any changes with your insurance plan. This is important for us to correctly submit claims and obtain authorizations for services. Co-payments and payments for services not covered by an insurance plan are due and payable at the time of service. We accept cash, check and most credit cards.

We require 48 – 72 business hours for prescription refills.  Please press option 2 and leave detailed information for us to properly handle your prescription requests.

Some insurance companies require a referral for a specialist visit or testing. Please do not schedule your appointment or test until we can confirm your authorization approval.

We require a 7 day notice to obtain an authorization for non-urgent requests.
