Diabetes Self-Management Training and Education Services

The Diabetes Self-Management Training Program offers patients with diabetes the necessary tools to facilitate diabetes self-management. Patients will learn vital lifestyle modifications, including healthy coping, healthy eating, being active, taking medications, monitoring, reducing risks and problem-solving.  Patients are entitled to 10 hours of initial diabetes self-management education in a 12 month period from the date of the 1st session, and 2 hours of follow-up per each calendar year.

According to the ADA 2022 Standards of Medical Care in Diabetes, patients should be referred for diabetes self-management training at these critical times:  at diagnosis, annually and/or not meeting treatment targets, when complicating factors develop, and when transitions in life and care occur. The Diabetes Self-Management Training Program offers 4 classes available in group, individual or tele-visit settings. Please discuss with your physician if you are interested in attending.  A referral from your physician is required to attend classes.

Pre-Diabetes

According to the CDC and American Diabetes Association: The sooner you know you have prediabetes, the sooner you can take action to reverse it and prevent type 2 diabetes. Prediabetes means your blood sugar is higher than normal but not high enough to be diagnosed with diabetes.

How To Reverse Prediabetes: The CDC-led National Diabetes Prevention Program helps people with prediabetes make lasting lifestyle changes to prevent or delay type 2 diabetes.
