In partnership with Lutheran SeniorLife, Heritage Valley Health System is proud to offer LIFE Beaver County and LIFE Lawrence County as an alternative to nursing home care. The LIFE program allows eligible seniors to maintain their independence at home with complete access to the medical, social and rehabilitation services they need. LIFE programs are administered through the Pennsylvania Department of Human Services and the Centers for Medicare and Medicaid Services (CMS).
Live LIFE at Home
Living Independence For the Elderly (LIFE) is an innovative managed care program that serves qualified residents ages 55 and older living in Beaver and Lawrence counties. It is an all-inclusive program of comprehensive medical care and support services that helps individuals live safely in their homes and lead independent and fulfilling lives. Nationally known as the Program of All-Inclusive Care for the Elderly (PACE), this model has proven to be successful throughout the United States.
LIFE is essentially a one-stop health plan for all of your medical, social and rehabilitation needs. LIFE Centers provide a single hub for primary care, pharmacy, nursing, physical and occupational therapy, adult day care services, care coordination and social work. The staff also coordinates additional supportive services, such as home health care, transportation, home accessibility modification, recreational programs and home-delivered meals. Based on need, additional services such as assistance with bathing, housekeeping, laundry and grocery shopping are also available.
LIFE coordinates and provides all preventative, primary, acute and long-term care services. Even if a participant requires hospital care, LIFE arranges and monitors the participant’s plan of care. This ability to coordinate the care of each participant enrolled in a LIFE program is key to the model.
