LIFE medical management services include the following:
- An interdisciplinary team including a physician, nurse practitioner, nurses, social workers, dietician, recreational coordinator and rehabilitation therapists
- An on call nurse and physician available 24 hours a day, seven days a week
- Individualized, personal care
- Support and assistance to caregivers and to family members involved in providing care
Based on an individual’s need, LIFE can provide the following services:
- Adult day services
- Personal care
- Homecare, housekeeping and laundry
- Recreational programs
- Physical, Occupational and Speech Therapies
- Transportation to the LIFE center and to medical appointments
- Medication management
- Meals/Assistance with meal preparation
- Medical equipment
- Equipment for safety in the home
- Dentistry, Optometry and Audiology
LIFE also manages the participant’s medical care in a hospital, nursing home or other inpatient or outpatient setting by coordinating appointments and the services received.
A person is eligible for LIFE if he or she is:
- 55 years of age or older
- Having difficulty managing his or her own health related needs independently at home and meets a level of care determined by the Beaver County Office on Aging.
- Eligible for Medicaid or able to pay privately
- Living in Beaver or Lawrence County
- Able to live safely at home with LIFE services
