Thomas Slokan, DO is board certified by the American Board of Family Medicine. He received his doctor of osteopathic medicine degree from the Lake Erie College of Osteopathic Medicine and completed a family practice residency at Saint Vincent Medical Center in Erie, Pennsylvania. Before joining Heritage Valley Medical Group, he served as medical director of the Saint Vincent Outpatient Center in Union City, Pennsylvania.

Dr. Slokan joined trinity Family practice in 2017. He is a member of the Pennsylvania Medical Society and the Beaver County Medical Society.
