Heritage Valley Medical Group Physicians Recognized for Diabetes Care

The National Committee for Quality Assurance (NCQA) recently recognized Heritage Valley Medical Group for its commitment to excellence in diabetes care.  93% of Heritage Valley Medical Group’s physicians met the standards for NCQA’s Diabetes Recognition Program (DRP).   The Diabetes Recognition Program (DRP) is a voluntary program designed to recognize clinicians who use evidence based measures and provide excellent care to their patients with diabetes.

“Several months ago, Heritage Valley completed a Community Health Needs Assessment where diabetes care ranked among the top community health concerns,” said Dr. Dan Brooks, vice president, community health services, Heritage Valley Health System. “Heritage Valley Medical Group physicians are committed to giving the highest quality care to those diagnosed with diabetes – NCQA recognition is evidence of that.”

The NCQA DRP program contains 11 measures including, HbA1c control, Blood Pressure control, LDL control, Eye examinations, Nephropathy Assessment, and smoking and tobacco use. To apply for recognition, physicians must abstract data from charts of diabetes patients and submit the information to NCQA for review.

Heritage Valley Medical Group is comprised of over 125 employed physicians located in 50 offices throughout Southwestern Pennsylvania and eastern Ohio.  It includes family practice and internal medicine physicians as well as specialists in cardiology, geriatrics, rheumatology, general surgery, cardiothoracic and vascular surgery, pulmonary, endocrinology, gastroenterology, pain management, hospitalist care, and other specialty care.
