  • The office is located inside Heritage Valley Beaver Hospital.
  • Park at Entrance A (Main Entrance).
  • Proceed past Outpatient Registration.
  • The office is on the left. There will be a green sign in the hall with the office and physician names. (Radiology will be on your right side.)

If you need a wheelchair, please ask staff at the Concierge Desk for assistance.

SEWICKLEY OFFICE:

  • The office is located inside Heritage Valley Sewickley Hospital.
  • Park in the Visitor Parking Lot.
  • Take the garage elevators to the 4th Floor.
  • When exiting the elevators, turn left to Pod A, which is another slight left.
  • Suite 403 is the first office. (It is located in the corner by the water fountain and women’s restroom.)

Wheelchairs are available on the 3rd Floor.

Medical History Questionnaire

Insurance cards and Photo ID are required for presentation at each visit to protect the identity of our patients.

We accept most insurance plans.  We will bill the insurance company for your visit.  However, all co-pays and self-pays are required at the time of service.  MasterCard, Discover, Visa, Checks,

and cash are all acceptable forms of payments.  Deductibles and co-insurance amounts will be the patients’ responsibility and payment arrangements can be made if necessary.

The physicians and staff at Heritage Valley Pulmonology and Sleep Medicine are pleased and proud to have the opportunity to serve the residents of the Beaver County area.

This practice is a combination of pulmonologists from two practices to form this organization to provide Beaver County with outstanding Critical Care, Sleep Medicine, and Pulmonary Disease Management.

We have a close working relationship with all of the Primary Care Physicians in the county for your best medical interest.

The office is located at on the first floor of Heritage Valley Beaver Hospital – right next to the Crazy Mocha coffee shop.

When hospitalization is necessary, our patients are admitted to the Heritage Valley Beaver Facility.

Office visits are by appointment only.  We participate with most major insurance plans.  Patients are responsible for obtaining referrals for their appointments.  Without the necessary referral, the appointment will be rescheduled.

MEDICATION REFILLS

For your safety and health, please inform the physician of any medication or change in medication that you take.  To ensure accuracy, please bring either your medication bottles or a complete list of your medications to include the name, the dosage (mg), and how often you take the medication.  Please also include any over the counter products you take.

Our physicians utilize electronic prescribing to send your prescriptions directly to your pharmacy.  This technology will help ensure the safety and accuracy of your medications.  You may request medication refills during your office or you may call the office if you need refills between your visits.  Routine medication refills will not be handled over the weekend by the doctor on call.

After-hours calls

One of our physicians is on call every evening and/or weekend for emergencies only.  This would include life-threatening circumstances or situations or sudden illnesses that might require immediate medical care.

Therefore, calls regarding routine medication refills or questions that do not require immediate assistance should be made to the office during regular business hours.

Please allow 48 to 72 hours to process medication refills.  Please make sure we have all updated insurance information at the time of refill request.  We will need to know if your request is a 30 or 90 day supply and pharmacy location.

AUTHORIZATION/REFERRALS

Patients are responsible for obtaining the referral from their primary care physician if the insurance requires.  The office is not responsible for calling the PCP at the time of service; your appointment will need to be rescheduled.  Authorizations for tests scheduled by the office will be obtained by the office staff prior to scheduling their test.

LATEX ALLERGY

Please notify our receptionist if you have or suspect an allergy to latex products.

Heritage Valley Pulmonology and Sleep Medicine provides Internal Medicine, including:

  • Sleep medicine – Identification / Treatment / Management
  • Pulmonary Disease – Identification / Treatment / Management
  • Critical Care
  • Ventilator Management

PHONE CALLS/MESSAGES

The office does have an automated system, only if all of the lines are busy.  You may be required to leave a message in a voicemail.  All calls are returned the same day by close of day.

APPOINTMENTS

Patients are seen in the office every day, from Monday through Friday.  We strive to see patients as close to their appointment time as possible.  If we are delayed in seeing you, please understand that due to the nature of the illnesses that we deal with, additional time can be required.

If you are unable to keep an appointment we ask that you notify us at least 24 hours in advance.  This enables the office to utilize the provider’s time as efficiently as possible.  There is a no show charge if you do not keep your appointment and have not canceled it.  Plan to arrive 10-15 minutes ahead of your scheduled time so we can register you into the computer and prepare you for your visit.  When you arrive for your visit, please sign in at the front window.
