The Community Health Services Department at Heritage Valley Health System offers individualized Medical Nutrition Therapy. A Registered Dietitian will provide an evaluation and work with you to create a healthful eating plan for a range of medical conditions, including:
- Diabetes
- Hypoglycemia (low blood sugar)
- Hypertension and high blood cholesterol
- Swallowing/chewing difficulties
- Obesity
- Poor Eating Habits
Fees:
Before your appointment, be sure to get the appropriate referral from your primary care physician. Your insurance provider will be billed, and you will be responsible for the balance of payment. If you have diabetes and private insurance in Pennsylvania, the insurer is required in most cases to cover medical nutrition therapy if prescribed by your physician.

You must obtain a written prescription from your physician with your diagnosis or nutrition concern. Schedule an appointment in advance. (Daytime, evening, and Saturday hours are available). After each visit, a brief summary of your appointment is sent to your referring physician. Follow-up visits are scheduled as needed and/or requested by you or your physician.
