Heritage Valley Health System performs Community Health Needs Assessments every three years. Below are the reports from prior years.
Heritage Valley Health System has conducted a Community Health Needs Assessment (CHNA), which focused on the health needs of Beaver County and select municipalities in the western section of Allegheny County.
The CHNA is based on a review of secondary statistical data, household telephone surveys, key informant interviews, and focus group discussions with health and human service providers and community residents. Representatives of Heritage Valley Health System and area health and human service providers have prioritized the top health needs as a result of this analysis. The top health needs identified during this process have become the basis for the development of the Heritage Valley Health System Implementation Plans.
The CHNA and Heritage Valley Implementation Plans to address the top health issues were approved by the Heritage Valley Health System Board of Directors on June 30, 2016.
